Registration Requirements for Players:
1. Completed
MYSA Registration Form,
click here to download this form - Coaches fill out this
form too.
2. Copy of the Players State Certified Birth Certificate
3. Signed & Notarized
Medical Release
4. Small school sized photograph of the player for the
MYSA Player Card
5. Registration Fees are $65 for the fall season and $50
for the spring season. The Head Coach is allowed 1
child/player or a player designee to play for free.

Player
Equipment
Uniforms (jersey & shorts) are provided
by Soccer West to the teams, and will be
returned to Soccer West at the end of
each season. Shin Guards are
required, and cleats are recommended,
but not required. Make sure if
your purchase cleats; they are soccer
shoes without a cleat at the end of the
toe. Baseball cleats are not
allowed as they have the toe cleat at
the end of the shoe. |

Sports City is where our office is located at. The
Soccer West Office location is where the sign-ups will
be conducted; and all paperwork will be collected and
returned to coaches. Sports City is not affiliated
with Soccer West, nor will any games be played at Sports
City. |

Coaches
Every coach must
have a State certified "F" license by the
second session
of his or her 1st coaching year. A year normally
starts in the fall, and ends in the Spring for a Coach. Coaches must
also apply for the
MYSA Kidsafe background check; click
here for the link to to apply for your Kidsafe
cerification number. The Kidsafe registration is
valid for 1 year from the time you submit your online
form. Coaches are all volunteers
that do not get paid; the head coach gets 1 child or
player fee waived (or free) per season, i.e. per fall
and spring season. |
